01 Step

Complete and Print Application Form DS-82

Click here to complete the application form online.

Completed application must have applicant’s original signature.

i Handwritten applications are not accepted. Application have to be completed online, and printed single sided.

02 Step

Passport

• Your passport should be issued within the past 15 years.

• You must’ve been over the age of 16 at the time your passport was issued.

• If your name has changed since your passport was issued, present a certified copy of legal document specifying your name change (e.g. marriage certificate, divorce, adoption decree, or court order)

03 Step

Passport Photo

You must provide one 2 x 2 inches passport photo. See U.S. Passport Photo Regulations.

04 Step

Letter of Authorization

Print, complete and sign TWO copies of the Passport Authorization Letter.

05 Step

Proof of Travel

You must provide a copy of ONE of the following :

• Printout of your foreign travel booking

• If this is a business trip, present a business letter from your employer

• If you are driving, provide your hotel reservation

06 Step

Fees

Government Fees

• Make a personal check or Money Order for $190 payable to U.S. Department of State. (Starter’s checks will not be accepted)

Our Fees

• Our Service Fee is $299

• For return delivery, choose one of the following:

* $30 for overnight shipping

* A prepaid filled in airbill from FedEx or UPS

* Use Your FedEx Account Number

* Pickup in person

• Credit card information should be entered on the service order form (see below).

• When paying by money order, make it payable to “Deluxe Passport Express”.

Complete the Service Order Form. Print a copy and send it along with rest of your documents.

Shipping

Visit one of our walk-in offices, or ship the following via FedEx/UPS:

1. Signed Application Form DS-82

2. Passport

3. Passport Photo

4. 2 Letters of Authorization

5. Travel Document

6. Government fee payment

7. Services Order Form

All signed documents must have original signature.
