Application Steps

Seven clear steps — we handle the complexity so you can focus on your travels.

01 Step

Complete & Print Application Form DS-82

The DS-82 must be completed online and printed single-sided. Handwritten applications are not accepted. Your original signature is required on the printed form.

i Do not sign the form until instructed — your signature must be witnessed by an agent.

02 Step

Proof of U.S. Citizenship

You must provide an original or certified copy of one of the following documents:

If you were born in the United States:

• Original Birth Certificate issued by city, county or state

• Previous undamaged U.S. Passport book or card

If you were born outside the US, click here to see your options.

i if you have been issued a passport book or a passport card within the last 15 years, it must be submitted for cancelation purposes, if its lost or stolen please follow instructions for lost passport.

03 Step

Proof of Identity

You must provide ONE of the following documents :

• Valid Driver’s License with Photo

• If the Driver’s License isn’t available, supply an alternate Proof of Identity.

04 Step

Passport Photo

Provide one 2 × 2 inch passport photo that complies with all U.S. Passport Photo Regulations (white background, taken within 6 months, no glasses).

05 Step

Letter of Authorization

Print, complete, and sign the Passport Authorization Letter. This authorizes Deluxe Passport Express to act as your registered agent and submit your application to the U.S. Department of State on your behalf.

06 Step

Proof of Travel

You must provide a copy of ONE of the following :

• Printout of your foreign travel booking

• If this is a business trip, you may present a business letter from your employer

• If you are driving, provide your hotel reservation

Complete the Services Order Form online. Print a copy and include it with the rest of your documents.

09 Step

Authorization

Prior to authorization, verify that you have collected the following documents:

1. Application Form DS -11 2. Proof of Citizenship

3. Proof of Identity 4. One Passport Photo

5. Passport Authorization Letter 6. Travel Document

7. Government Fees

• You must appear in person at a post office or court house and present the original documents to an acceptance agent, which is authorized to accept passport applications. The acceptance agent is needed to witness the signing of your application for the passport.

• Notify the clerk you are using an expediting service to “hand carry” your application. Most facilities are open M-F, 9am to 330pm.

• The clerk will ask you to sign the DS-11.

• The clerk will then seal the DS-11, photo, proof of citizenship, ID, travel document, authorization letter and the $190 check in an envelope marked “To be Opened by U.S. Passport Office Only”. Make sure the the agent puts everything in.

• Important: Make sure the clerk returns the sealed envelope to you.

i Do not open this sealed envelope or the application will be invalid.

10 Step

Shipping & Submission

Visit one of our walk-in offices, or ship the following via FedEx/UPS:

• Sealed Government Envelope

• 1 extra Passport Authorization Letter

• Services order form

Call us at 718-387-5441 to inform us that you’re processing an Emergency Passport. Overnight it to Deluxe Passport Express, 57 Lee Ave, Brooklyn NY 11211.
